Home Health Services

At Park Ridge Home Health we recognize that home care is not just a method of service delivery. It is active support compassionately delivered in the comfortable and familiar setting of the patient’s own home.

Our trained professionals provide coordinated services to homebound, seriously ill or injured patients who need skilled care. Upon referral, a nurse or therapist will visit with the patient to assess his or her needs and to develop an individualized care plan. Under the direction of a physician, a care team will then deliver the necessary services using the latest techniques aimed at maintaining or restoring patient independence.

    In addition to providing physical care, Park Ridge Home Health offers spiritual counseling that enables patients to live their lives with dignity, quality and spiritual healing.

    The Park Ridge Home Health services are available 24 hours per day, seven days per week. Administrative offices are open Monday – Thursday from 8 a.m. to 5 p.m. and on Friday from 8 a.m. until 4:45 p.m. For more information, call 828-687-5261.

    Answer: If a client has either Medicare or Medicaid and qualifies for Home Health, the services are paid at 100%.     Answer: If you have a question about meeting criteria, call (828)687-5261 and ask to speak with an Intake Nurse. He or she can discuss criteria with you over the phone, or we can mail you the necessary information.
